What

The DIALOG assessment consists of 11 questions that enable individuals to assess their satisfaction in eight life domains and three treatment aspects using a 7-point scale. This evaluation provides scores for both subjective quality of life and treatment satisfaction. While the scale is a key part of the DIALOG intervention, it can also be used independently.

PKB has digitized the DIALOG and DIALOG+ questionnaires for teams to use with patients. These digital questionnaires can be completed directly from a patient's PKB home screen or sent by a healthcare professional or coordinator before or during an appointment.
