This section of the care plan toolkit contains all the care plans created by PKB for patients to monitor and track symptoms related to their disease or disorder. Please browse through the templates and inform your Success PM about which one your team would like to use with their patients.
Any care plan in this section can be customized to fit the needs of your patient group and team's speciality.
What’s contained in a PKB care plan?
PKB will always show any diagnoses, allergies and medications that have been entered into the patient record. The main body of the plan includes an action plan. The action plan can contain information on the patient's care and health goals, as well as advice on monitoring their conditions. (Including video or web links). Key Symptoms and Measurements can also be selected to track and display for the patient.

Maternity High Blood Pressure care plan: Giving expectant mothers the ability to track BP and symptoms

Bladder Diary care plan: Giving patients a place to record their daily in and outputs and the ability to share with the healthcare teams looking after them
Blood Pressure Home Monitoring care plan: allowing patients to track, record and self-monitor their blood pressures at home

ADHD care plan: a generic care plan on ADHA that can be added to any live site to support people
Hypertension plan: a generic care plan on hypertension and high blood pressure that can be added to any live site to help patients self-manage
Asthma care plan: a generic care plan on asthma that can be added to any live site to support patients with diabetes to self manage their condition
