To provide patients with digital care plans with information on high blood pressure and hypertension. All information taken from https://www.nhs.uk/ and NICE guidelines.

Outcome

Equip patients with the information they need to self-manage their high blood pressure. provide generic disease care plans for teams to have in their PKB site for patients to engage with while waiting fo treatment or after diagnosis.
