How do I create

questionnaires?

Advanced questionnaires are built and maintained by PKB internally. Therefore, if your team need a new advanced questionnaire built, please contact your Customer Success PM. If your team has not yet moved to advanced questionnaires, please read on to find out how to build a questionnaire template.

Create a spreadsheet (CSV) and enter the questions and answer options. Each row in the spreadsheet is one question in the consultationquestionnaire

The first column is the text to be displayed (for example, What is your name? or How do you feel?)

The third column determines whether or not the question is mandatory.  Setting the value to TRUE will make the question mandatory, whilst leaving it blank or setting it to FALSE will make the question optional. If a user attempts to submit a consultation questionnaire without answering all of the mandatory questions, they will be shown an error message and the mandatory questions that they have not answered will be highlighted.

Please note if you are going to include commas in any spreadsheet cell, you must surround all the text in the cell by quotes. So for example, if you wanted instead of entering Hello, do you like my tutorial? you should instead enter 'Hello, do you like my tutorial?'

Professionals and coordinators are able to extract data from consultations questionnaires into a CSV file. Each report contains every instance of a completed questionnaire, with a row for each set of responses and columns for every question.

How do I send a

questionnaire to all patients in my team?

To send a mass consultationquestionnaire, click ‘Send consultation’ questionnaire’ on the consultation Questionnaires page, select the correct consultation questionnaire and click ‘Start’ to send this to all patients. 

The consultation questionnaire will be sent to all patients who are registered or have an email address in their record but have not yet registered.

How can I see hidden and visible

questionnaires?

Professionals and coordinators can see the list of visible and hidden consultations questionnaires listed on the Consultations Questionnaires page. 

Hidden consultations questionnaires will no longer appear for professionals and coordinators when they attempt to send out an individual or mass consultationquestionnaire.

And patients will no longer see any hidden consultations questionnaire when they start initiate a questionnaire by clicking ‘Start consultation'.

The responses for all consultations questionnaires which have been completed are still available for download for both visible and hidden consultationsquestionnaires.

A warning will be given to the patient indicating that the consultation questionnaire is no longer available for completion.
