When creating a care plan in PKB for your patients, it's useful to use the template provided, making sure you utilise all the functionalities and know how patients can interact with it once it's been created.

PKB has put together the care plan template document below, where teams can either download or copy the template and add the content.

After adding the content to the care plan template, share it with your Success project manager to make sure every section has been utilised and any blood test added have the right values.

The care plan template can be submitted using the PKB care plan request form.
