Key objectives

To use PKB to facilitate remote communication and monitoring with patients at home following surgery.

Outcome measures

To equip patients with a tool to support self-management of their health and well-being, as well as providing a facility to interact digitally rather than relying on face-to-face questionnaires.

Current Baselines

All questionnaires are completed face to face on paper and patients do not get copy to take home.

Workflow

1. Team: asks patient to complete questionnaire

2. Patient: completes questionnaire and submits to the team for review

3. PKB: notifies the team, the questionnaire has been completed

4. Team: reviews questionnaire in appointment with the patient
