Key objectives
Provide patients with a digital consent process, enabling informed consent before their procedure. Patient's are given the right information digitally by the team allowing for remote care before they arrive at the hospital.
Outcome measures
Equip patients with the information they need before their procedure, providing them with a digital consent form that can be done remotely before their procedure, empowering patients to have informed consent.
Current Baselines
All consent forms are completed on paper in person prior to the patient's procedure, information is explained and the doctor goes through a paper form with the patient, appointments aren’t being efficient and patients aren't always giving informed consent.
Workflow
Team: Adds consent care plan to the patient's record
Team: Fills in the consent care plan
Team: Sign the consent care plan
PKB: notifies the patient of the saved care plan
Patient: Logs in and sees care plan
Patient: Signs consent care plan
Team: Sees patients consent before the procedure/operation and confirms consent
